Fill out the form below and click the submit button. The form is for both Issuing and Returning UU iPads.

Please follow the steps below to accept an iPad from a model.
CHECK THE PASSWORD BY LOCKING THE IPAD AND THEN SIGNING BACK IN. Do this BERFOR THE MODEL LEAVES.
MAKE NOTE OF THE PASSWORD AND THEN RESET IT TO 1234 . (This is the password given to all iPads that are not assigned to a model)
Check the iPad for cracked screen. Make notes below of any significant damage. All iPads have wear and normal scuffs and scratches but the screens are all in good condition.
If the iPad is damaged or non functioning upon returning it the repair or replacement costs will be added to the models account. The accounting department will inform the model of any costs. DO NOT DISCUS ISSUES RELATED TO REPAIRS. refer them to accounting.

Place the iPad in the the assigned iPad storage location in the agency.
